Notice of Intent to Teach Public Education Course

This web-form of our ANSC-7023 will allow you to post your class and file your Intent to Teach form. The below includes all the necessary information but in a simpler format. If you have more than one course you are reporting, then complete the form for the first course and hit [Submit] and repeat for each course. This form will be sent to the DSO-CS for verification, to the DSO-PE, State Reps, and BoatUS.. NOTE the class will be posted with the content you submit. Please be complete! In order to submit a course you must be logged in to the site. If you don't have an account register here and then please email the DSO-CS to obtain permissions.

IMPORTANT NOTICE - Effective May 1st D13 will be transitioning to the National PE database. Classes which occur after May 1st should be posted to the National PE Database. Classes prior to that should be posted using this form.

All persons whose names and contact information are utilized in the connection with this form expressly authorized the dissemination and publication of the information to members of the general public. The person completing this form acknowledges that the appropriate permissions have been obtained.

Times should be entered in as follows 7:00 PM or 7:00 AM. Please include the space in between. Additional information might include a description of a class that does not fit any description or standard code offered such as (BCN, ACN, etc.). Use the remarks block to indicate courses not on the list of classes.
